clutch spacing

So I just got my banshee running, but everytime I let out the clutch it dies. I took apart the clutch side of the motor and noticed that a fiber plate had come out of the basket and jammed everything up. Its adjusted correctly. I'm just lost as to why the plate keeps coming out. I just ordered a new clutch, lock nut washer, a new spindle, and a new gasket. But I'm just curious what might be the issue.

Shot in the dark here, pressure plate not seated correctly with arrows lined up? So there is a gap allowing the last plate to come out?

If the pressure plate is lined up, seated all the way and the bolts inside the clutch springs on the pressure plate are tight. I don't really see how a fiber could come out.
