On going overheating problem

It sure does sound like a head gasket, drain coolant and pressure test coolant system. A lot of people take for granted how clean everything needs to be on a reassemble.chrck cylinder tops and head for grooves caused by previous gasket installs. I glass bead all surfaces it really gives a uniform finish and enables u to really see wtf is going on. Everytime I do anything to an engine I change water pump seal and check pump and gear . Stop driving it like that ide be really suprised if the piston skirts didn't collapse or have scoring on cyl walls

Could be a multitude of things. But none the less you have a motor that isn't sealed up. I would start with a simple pressure test on the coolant system. Then do a leak down. The high idle and smoke out the right could be a bad gasket or pto side seal. I'll bet that thing is probably eating coolant and has an air leak.
